Prolactinoma – The most common Pituitary Adenoma & its Types

The pituitary adenoma has got the commonest type of tumor called the prolactinoma which is a benign adenoma. The prolactinoma secretes from the pituitary gland, a hormone called prolactin in excess which is a natural hormone supporting woman’s breastfeeding. The normal function of the prolactin is to stimulate the breast tissues so that they are enlarged during the time of the pregnancy.

This hormone is produced by the pituitary gland which is also the master gland of the human body and plays a vital role in promoting growth and development, reproduction and the metabolism in the body, and also in the production of various hormones including prolactin. Any malfunctioning in the pituitary gland starts leading to the formulation of the pituitary adenoma.

The pituitary adenoma may cause a fluctuation in one or more pituitary hormones damaging the normal tissues and enlarging the pituitary gland. Prolactinoma is the most common type of the pituitary adenomas, where the clinically significant pituitary adenomas affect the conditions of the people by approximately 1 in about 7000 people.

According to recent studies, about 25% of the people in U.S. have pituitary tumors which are small in size, of which 40% of these tumors secrete prolactin but which are not clinically significant.

The prolactinomas are categorized into two types- the microadenomas (less than 1 cm) and the macroadenomas (1 cm in size). It is based on these sizes that the symptoms for the pituitary adenoma vary in different patients calling for different treatment options or therapies to deal with problem of the prolactinoma of the pituitary adenoma.





Giant Pituitary Adenomas with Postoperative Pituitary Apoplexy

One of the main causes for the partial resection of the giant pituitary adenoma can be the presence of the postoperative pituitary apoplexy. The pituitary apoplexy is a very rare case which is not only life threatening but is actually characterized by symptoms like headache, opthalmoplegia, visual loss or even a disturbed mental status resulting from the prompt hemorrhage or the malfunctioning by the pituitary gland.

The postoperative pituitary apoplexy or the massive swelling and hemorrhage in the pituitary adenoma, is a complication that is not much reported.

Doctors specify that there can be various reasons due to which pituitary apoplexy occurs. The most proposed reason is the drastic growth of the pituitary adenoma outstripping the blood supply and leading to the ischemic necrosis of the pituitary gland. S

ome other authors specify that it could also be due to the straight compression of the pituitary infundibulum extending itself as a mass while suppressing the blood flow from the portal vessels which would end up in the necrosis of the whole pituitary gland and thus giving way to hemorrhage.

There could also be a third possibility where the internal carotid artery releases tumor vessels imminently because of the reduced tumor burden and the tumor’s venous drainage during the surgery. However, the angiographic studies reveal that the pituitary adenoma does not get its blood supply from the superior hypophyseal arteries and instead gets its blood supply from the inferior hypophyseal arteries.

The most viable suggestion is to attempt a radical tumor resection at the time of the first surgery of the giant pituitary adenoma which in itself is a big challenge for the surgeons involved in it. This is because the mortality rate is of a higher degree in this phenomenon and the implications are also high leading to a pituitary apoplexy.





Juvenile Pleomorphic adenoma – In the Cheek of a Child

The pleomorphic adenoma which is the commonest form of tumor of the salivary glands affects women and children more than the adult males. Though 90% of these tumors are found in the parotid glands and 10% of them in the minor salivary glands, the pleomorphic adenoma of the minor salivary glands is usually formed in the soft palates of the lips and cheeks.

The other regions include the pharynx, nasal cavity, throat, tongue, tonsil, retromolar region, etc. But the intraoral pleomorphic adenomas are hardly found in children, but a thorough examination of the tumor and its treatment is mandatory.

In the case of the juvenile pleomorphic adenoma, the tumor normally affects the major salivary glands, but in the children, the case of the tumor in the minor salivary glands in the cheek has rarely been reported. The various diagnoses of the juvenile pleomorphic adenoma in the cheek region show that there are different kinds of the adenoma stages which comprise of the dermoid cyst, fibroma, lipoma, buccal space abscess, mucoepidermoid carcinoma, neurofibroma, foreign body reaction, carcinoma ex pleomorphic adenoma, adenoid cystic carcinoma, and polymorphous adenocarcinoma.

This differential diagnosis needs a high level of observation and manifestation to detect the presence of the pleomorphic adenoma in the cheek. The treatment that is highly recommended to deal with this tumor is the complete wide surgical excision, but doctors say that the recurrence of the tumor is of more concern to them, as the malignant transformation of the tumor can become manifested if there is no proper follow up post surgery. A pleomorphic adenoma of the accessory lacrimal gland is basically a rare case and has presented a more refined meaning to denote the benign mixed tumor that was earlier denoted as the lacrimal gland tumor consisting of both the epithelial and the stromal elements. The huge size, the clinical resemblance of the tumor to the eyelid and the bone adenomas, actually make the pleomorphic adenoma of the lacrimal gland a noticeable case.

The giant pleomorphic adenoma which is a mixed lacrimal tumor arises from the ectopic lacrimal tissue which is also observed in the eyebrows, upper eyelid and orbit and has no relation with the lacrimal gland. And the pleomorphic adenoma related with the glands of Wolfring can present as lid tumor because of its presence at or near the upper lid.

But a patient with pleomorphic adenoma of the accessory lacrimal gland producing from the Wolfring glands can also have the possibility of having the tumor from the Krause’s glands if the globe’s downward displacement is not extended to a maximum degree.





Treatment of Adenoma Plural

Some of the common forms of adenoma cyst include serocyst adenoma, mucinous cyst adenoma intraductal papillary mucinous cysts and many more. These cysts are mostly available in benign form and are non inflammatory. The serocyst adenoma is commonly found in middle aged women and is known as benign serocyst adenoma. This form of adenoma is localized and found in the posterior structure of the pancreas. In some cases, the women report a mild to severe abdominal pain, but there were complications associated with the benign serocyst adenoma.

Adenoma histology is a term used to describe the detailed analysis of the adenoma tumors that are found in the glandular tissue in the various endocrine glands. Today, in spite of the advent of technology in the medical arena, large numbers of people are still dying from cancer. Cancer is available in various forms, it can be found in the form of a benign adenoma or a malignant adenoma. The worst part about cancer is that it is a silent disease, which does not produce any significant symptoms until the disease has progressed to the final stages.
